THE GRAMM-LEACH-LILEY Act of 1999 requires that all insurance service companies must treat non-public personal financial information as confidential. In requesting the services of your firm, to assist us in meeting our obligation to our customers, we may disclose to you items of personally identifiable non-public financial information or health information to enable you to perform the function requested. We advise you that we expect you to maintain complete confidentiality of that information. It may be used for the purpose of performing the function we have requested and for no other purpose. If the information is retained in your files, following the completion of this assignment, you are obligated to maintain its security and confidentiality. The information may not be disclosed to any party other than our company or as permitted by law.

US-Reports, Inc. considers maintaining the confidence and trust of our customers to be our first priority. The company strives to protect the confidentiality and security of non-public personal information about our customers and their policy holders that is presented to us in the normal course of business. In light of the growth of computer technology, the development of internet commerce and the multiple changes in communications, the need for security of information becomes increasingly important to all of us.

THE UNITED STATES CONGRESS passed the Gramm-Leach-Biley Act in 1999. A portion of that act addressed the manner in which insurance related companies must treat non-public personal financial information that is in their records. It also requires that we provide our staff and contracted firms and individuals who have obtained our services for personal, family or household purposes, with a statement explaining our privacy policy and practices.
US-Reports, Inc. gathers information to complete it's clients assignments from various sources, including:

• Information that we receive from policy holders or other forms completed related to the assignments.
• Information about transactions relating to policy holders insurance business.
• Information that we receive from outside sources, such as BEST Insurance guides, industry "links" such as county assessors offices, OSHA, PAAS, etc.. in gathering pertinent information relating to the policy holder's insurance business.
• Information provided to us by policy holder insurance agents.

WE DO NOT DISCLOSE ANY NON-PUBLIC PERSONAL INFORMATION ABOUT OUR CUSTOMERS OR FORMER CUSTOMERS OR THEIR POLICY HOLDER'S TO ANYONE, EXCEPT AS PERMITTED BY LAW.

THE INDEPENDENT FIELD REPRESENTATIVES who are authorized by contractual arrangements to complete assignments of our clients are not employees of our company but are subject to comply with our privacy policy. These representatives may have additional or different privacy practices as well.

SECURITY FROM UNAUTHORIZED ACCESS to our clients and their policy holder's non-public personal information is a major concern of US-Reports, Inc.. We maintain physical, electronic, and procedural safeguards to protect this information from being accessed by unauthorized persons.
